Perimenopause is having a cultural moment, and endocrinologist Dr. Komal Patil-Sisodia is glad it is, but in this launch episode of Clearly Hormonal, she pushes back on social media's habit of attributing nearly any midlife symptom to hormones. She defines perimenopause as the years leading up to menopause (recognized after 12 months without a period, average age ~51) and explains that it's marked by hormonal variability, not simply “low estrogen,” with menstrual-cycle changes serving as an important clinical clue.

She walks through why most women don't need single-point hormone testing, since levels fluctuate day to day, per current ACOG guidance. Using a composite patient example (“Jen”), she shows how fatigue, sleep issues, hair loss, and weight gain may reflect iron deficiency, thyroid disease, sleep apnea, nutrition, stress, medications, or life changes, not hormones by default.

She closes with a four-question framework for sorting it out: where you are in the reproductive transition, whether a symptom is plausibly related to it, what else could explain it, and whether treating perimenopause would actually help. Taking symptoms seriously, she argues, requires evidence and continued curiosity, not a single convenient explanation.

If you're a woman in your 40s and you spend any time online, you've probably seen some version of this: you're exhausted? Perimenopause. You're gaining weight? Perimenopause. You can't sleep, you're anxious, your hair is thinning, your joints ache, you walk into a room and forget why? Perimenopause, perimenopause, perimenopause.

I want to say something that might sound contradictory: I'm thrilled we're finally talking about this. For decades, women weren't given the language for what was happening in their own bodies. Symptoms were dismissed. Women were told they were too young, or that they'd just have to deal with it. That silence was the real problem, and the fact that we're talking now is genuine progress.

But we've swung from not talking about perimenopause enough to sometimes treating it as though it explains virtually anything that happens to a woman between 40 and 55. And that's not necessarily better medicine. Fatigue can also be iron deficiency. Hair loss can also be thyroid disease. Poor sleep can also be sleep apnea. Just because a symptom shows up during perimenopause doesn't mean it's happening because of perimenopause.

What Perimenopause Actually Is

Perimenopause is the transition leading up to menopause, and menopause itself is one point in time: your final period, recognized in retrospect after 12 consecutive months without a period. The average age of menopause in the U.S. is around 51; perimenopause commonly begins in the 40s, though I've seen it start as early as the 30s and as late as the early 50s.

Here's the piece that gets lost most often: perimenopause is not simply a state of low estrogen. In the early transition, hormone production becomes more variable: ovulation becomes less predictable, and estrogen can be relatively high one point in the cycle and low at another. That variability is a big part of why symptoms can feel so unpredictable. One month you feel fine; the next, you're wide awake at 3 a.m. wondering what happened to your body.

One of the clearest clues that someone is entering the transition is a change in their menstrual cycle: cycles getting shorter or longer, periods being skipped, flow changing. That doesn't mean you can't have symptoms before your cycle becomes obviously irregular, but menstrual history is an important part of the clinical picture, and it's exactly what gets lost when perimenopause gets reduced to an all-purpose symptom checklist broad enough that almost anyone in midlife could check several boxes.

Why a Single Hormone Test Rarely Settles It

Understandably, people think: if we're trying to figure out whether this is hormonal, why not just measure the hormones? But for a typical woman in her 40s with characteristic menstrual changes and symptoms, a single FSH or estradiol level often won't give a definitive answer, because these hormones fluctuate; measured on two different days, you can get two very different results.

Current ACOG guidance is direct about this: most women don't need hormone testing to determine whether they're in perimenopause. Age, symptoms, and menstrual changes usually tell us more. Testing becomes more relevant when symptoms or menstrual changes occur at a younger age, or when the clinical picture is atypical.

Sometimes the more useful question isn't “What's my estrogen?” It's “Is there another reason I feel this way?”

A Composite Patient: “Jen”

To make this concrete, consider “Jen,” a composite, not a real patient, built from conversations I have on repeat. Jen is 44 and comes in exhausted, gaining weight, with thinning hair and poor sleep, convinced it's all perimenopause.

Her cycles are still fairly regular and she has no hot flashes or night sweats, useful information on its own. Fatigue and poor sleep without vasomotor symptoms send me looking elsewhere first: we check iron, thyroid studies, and talk through her sleep in detail. It turns out she's waking at 2 a.m. most nights, snoring, tired no matter how much she sleeps, a sleep apnea story, not a hormone one. Her hair thinning tracks almost exactly with a significant stretch of weight loss eight months earlier: a nutritional and shedding pattern, not androgenetic hair loss. And her weight changes track with a new desk job and a knee injury that stopped her usual workouts, not a mystery, just a life change.

None of this rules out perimenopause down the road. It just means that on the day she walked in, “it's perimenopause” would have closed four different doors that needed to stay open.

Symptom by Symptom: What the Evidence Actually Supports

Fatigue can absolutely be worsened by night sweats and menopause-related insomnia, but it's also nonspecific: iron deficiency, anemia, hypothyroidism, sleep apnea, depression, medications, and nutrition can all cause it. Fatigue is a symptom, not a diagnosis.

Weight gain deserves its own episode (coming soon), but the short version: real changes in body composition and fat distribution happen across midlife, layered on top of aging, activity, sleep, and metabolic factors. “My body composition has changed during this transition” is reasonable; “my estrogen dropped and that's why I gained 20 pounds” is a much more specific claim than the evidence usually supports, and menopausal hormone therapy is not a weight-loss medication.

Brain fog, anxiety and mood changes, hair loss, sleep disruption, low libido, and joint pain all show up on the internet's perimenopause lists, and all of them have their own differential. Sleep, stress, thyroid disease, iron levels, medications, relationship and caregiving load, and life circumstances all belong in that conversation alongside hormones.

Giving a symptom a hormonal label isn't the same thing as explaining it.

When “It's Just Perimenopause” Becomes Its Own Problem

“It's just perimenopause” can become just as dismissive as “it's just stress” once used as a stopping point rather than a starting one. Abnormal bleeding, persistent palpitations, severe unexplained fatigue, significant unexplained weight change, new neurologic symptoms, marked hair loss, new or severe headaches, or symptoms that seem unusually early for the transition all deserve a closer look rather than an automatic label.

Perimenopause doesn't grant immunity from every other medical condition. You can be in perimenopause and have iron deficiency, hypothyroidism, sleep apnea, depression, or diabetes. Two things can be true at once, and good medicine keeps that possibility open.

The Other Side of the Pendulum

None of this means perimenopause isn't real, or that women should have to prove their symptoms before being taken seriously. You shouldn't have to wait for 20 hot flashes a day, or be told you're too young because you're still menstruating, or produce a dramatically abnormal lab value to be believed. The goal isn't skepticism toward women; it's skepticism toward overly simple explanations. There's a middle ground between “everything is perimenopause” and “none of this could possibly be perimenopause,” and that's where thoughtful medicine actually lives.

A Four-Question Framework You Can Use

  • Where am I in the reproductive transition? Age, cycle changes, hot flashes or night sweats, contraception, hysterectomy history: the whole picture, not one symptom.

  • Is this particular symptom plausibly related to perimenopause? Vasomotor and vaginal symptoms have the strongest evidence base, per ACOG; others are less clear-cut.

  • Is there another common, or important, explanation to consider? Not 47 blood tests, but a real look at the alternatives.

  • If this is happening during perimenopause, would treating perimenopause actually improve it? Association is not the same as causation, and causation is not the same as an indication for treatment.

The Bottom Line

Perimenopause is real, and its symptoms are real. But taking women seriously doesn't mean attributing everything to their hormones; it means staying curious enough to ask what else could be going on, whether the explanation actually fits the physiology, and whether the treatment being recommended has evidence behind it for this particular problem. That's the whole spirit of Clearly Hormonal: taking hormones seriously, without making hormones the answer to everything.
