If you've seen Dr. Komal Patil-Sisodia at another practice within the past 3 years, we're delighted to welcome you to Eastside Menopause & Metabolism.

Because Dr. Patil-Sisodia is already familiar with your medical history and prior treatment, you may qualify for a Returning Patient Initial Visit rather than a New Patient Consultation. This appointment is designed to make your transition to our practice as smooth and seamless as possible—so you can continue your care without starting over.

Who Qualifies?

You may schedule a Returning Patient Initial Visit if:

• You were previously evaluated or treated by Dr. Patil-Sisodia at another practice.

• Your most recent visit with Dr. Patil-Sisodia was within the past 3 years.

• You are returning for continued endocrine, menopause, metabolic, obesity medicine, or related care.

If it has been more than 3 years since your last visit, or if you have never been seen by Dr. Patil-Sisodia before, please schedule a New Patient Consultation.

What to Expect

Your Returning Patient Initial Visit provides dedicated time to reconnect, review any changes in your health, and update your treatment plan.

Before your appointment, we'll review your previous medical records and update your medical history. If your records are not already available, our team will help obtain them with your authorization.

During your visit, Dr. Patil-Sisodia will:

• Review your current health concerns and goals.

• Update your medical history, medications, and supplements.

• Discuss any changes since your last visit.

• Review recent laboratory results or imaging, when applicable.

• Develop an updated, personalized treatment plan based on your current needs.

Will I Have to Start Over?

No.

While we'll take time to review what's changed since your last visit and update your medical information, you won't be starting from scratch. Dr. Patil-Sisodia already knows your medical history, previous treatment approach, and the goals you've been working toward.

That means your appointment can focus on moving your care forward—not repeating the past.

Returning Patient Visit Fee

Returning Patient Visit: $350

This visit includes the additional time required to review prior records, establish your chart within our practice, and provide a comprehensive evaluation tailored to your current health needs.

As a direct specialty care practice, Eastside Menopause & Metabolism does not bill insurance. Payment is due at the time of your visit. If you'd like to seek possible out-of-network reimbursement, we're happy to provide a superbill upon request.

Before Your Appointment

To help us prepare for your visit, please:

• Complete your intake forms through our secure patient portal.

• Sign the authorization form if we need to request records from another healthcare system.

• Upload any recent laboratory results or imaging that may not already be available.

• Bring an updated list of your current medications and supplements if needed.

Providing recent records before your appointment helps us make the most of your visit.
