<image:title>Clearly Hormonal - 18: The Power of Your Internal Clocks: How Circadian Rhythms Shape Our Health with Lynne Peeples - Welcome to Clearly Hormonal (formerly Reset Recharge)</image:title>
<image:caption>EP. 18 For many women in midlife, sleep struggles, low energy, and weight changes can feel like an inevitable part of menopause. But an often overlooked culprit is circadian disruption. Hormonal shifts and the pace of modern life can throw off our internal clocks—the rhythms that regulate nearly every system in the body, from metabolism to mood. When these clocks fall out of sync, it doesn’t just affect sleep. It can also disrupt hormone balance, increase the risk of chronic disease, and leave you feeling out of sorts without a clear reason why. And most of us have no idea how much these hidden clocks influence our daily lives—or how to get them back on track. I recently spoke with science journalist Lynne Peeples, author of The Inner Clock: Living in Sync with Our Circadian Rhythms, who shared how menopause impacts circadian function and the powerful strategies she uses to restore rhythm and improve overall health—starting with small, daily shifts.</image:caption>

<image:caption>EP. 17 For many women, hair is a big part of their identity, femininity, and confidence. When hair loss occurs, it can feel worrying, especially when the cause is unclear and the path forward is unknown. Whether it’s hormonal changes from pregnancy, perimenopause, or menopause, many women struggle to understand what’s happening and how to address it. I recently met with Dr. Echefu, a hair transplant surgeon, who shared invaluable insights on the different types of hair loss women face, from scarring to non-scarring alopecia and shared her approach to addressing it through personalized treatment options. Keep reading to learn more about the causes of hair loss and the practical steps you can take to start addressing your hair health today.</image:caption>

<image:caption>EP. 16 We’ve all put off a doctor’s visit—whether because we “feel fine,” life gets hectic, or scheduling an appointment feels like just another task on a never-ending to-do list. I know I did. After having my son, five years passed before I finally made it to a doctor. But many of the biggest health concerns for women don’t come with obvious symptoms right away. By the time we feel something is wrong, it may have been developing for years. Recently, I sat down with Dr. Lindsay Ogle, a board-certified family medicine and obesity medicine physician, who broke down the key screenings women need at every stage of life. In this post, I’ll share what I learned—why these screenings matter, how often to get them, and how to prioritize your health even when life feels overwhelming.</image:caption>

<image:caption>EP. 15 Menopause isn’t a blink-and-you-miss-it moment. It’s a major life shift, and yet, so many of us wait until symptoms hit to pay attention. The reality is that this transition is a long journey that spans your lifetime. For some women, perimenopause can last anywhere from five to 10 years—and sometimes even longer. The earlier you start preparing, the more control you have over your experience. By taking a proactive approach, you can better manage your symptoms and avoid feeling overwhelmed when they arrive. In this post, we’ll explore 10 key strategies that will help you navigate perimenopause and menopause with confidence, from tracking symptoms and optimizing sleep to managing metabolic health and knowing when to seek treatment. Let’s dive into how to advocate for your health, reflect on your needs, and ensure you’re prepared to handle this life transition on your terms.</image:caption>

<image:caption>EP. 14 For many women, poor sleep has become a normal part of life, whether due to pregnancy, menopause, family demands, or career pressures. In my twenties, I could fall asleep anywhere, but as I’ve gotten older, sleep has become much more elusive. So, how do we reclaim restful sleep as our bodies change, and how much sleep do we really need to feel refreshed? Sleep issues during menopause are incredibly common, yet they often go unaddressed. As our hormones shift, so do our sleep patterns. The good news is, there are effective strategies to help us sleep better during this phase of life. In this post, we’ll explore how hormonal changes affect sleep, why sleep is essential for our overall health, and most importantly, how to improve sleep and reclaim rest in menopause. If you're struggling with sleep during menopause, keep reading for practical tips and advice on how to sleep better and thrive through this season of life!</image:caption>

<image:caption>EP. 13 Polycystic Ovary Syndrome (PCOS) is more common than many realize, yet it’s so often misunderstood and misdiagnosed. Frequently called a "disease of exclusion," PCOS is commonly diagnosed only after ruling out other conditions, leaving women to suffer with symptoms like irregular periods, weight gain, acne, and fatigue for years. This delay can lead to serious complications such as obesity, type 2 diabetes, and cardiovascular disease. Early diagnosis and treatment are crucial for managing symptoms and reducing long-term health risks. In this post, we explore the causes and symptoms of PCOS, its connection to metabolic syndrome, and how revolutionary treatments like GLP-1 medications are changing care. We’ll also look at how PCOS evolves through perimenopause and menopause and why ongoing management is key to lifelong health.</image:caption>

<image:caption>EP. 12 Previously, we explored insulin resistance, a condition where your body struggles to use insulin effectively, often worsened by hormonal shifts during perimenopause. Now, we turn our attention to prediabetes—a stage where blood sugar levels are elevated but not yet high enough for a type 2 diabetes diagnosis. While prediabetes increases the risk of developing type 2 diabetes and cardiovascular disease, it doesn’t have to be inevitable. Early intervention is key. Today, we’ll discuss how simple lifestyle changes—like adjusting your diet and exercise—can reverse prediabetes, regulate blood sugar, and lower health risks. We'll also cover medical treatments, including anti-obesity medications and hormone replacement therapy, to further support metabolic health.</image:caption>

<image:caption>EP. 11 Struggling with stubborn pounds around your midsection, despite sticking to a healthy diet and regular exercise, is a frustrating reality for many women as they enter perimenopause and menopause. Hormonal shifts—particularly a decline in estrogen—can trigger insulin resistance, making it harder to lose weight, control blood sugar levels, and maintain muscle mass. In this episode, the first of my Endocrine Series, we dive into what insulin resistance is, how decreasing estrogen during perimenopause worsens it, and the key steps you can take to monitor and manage it effectively. Tune in for practical tips to help you work with your healthcare providers and take charge of managing insulin resistance, weight changes, and overall metabolic health during perimenopause and beyond.</image:caption>

I recently had the pleasure of sitting down with Dr. Susan Baumgaertel, a seasoned internal medicine physician with over three decades of clinical expertise, who has pivoted her career to focus exclusively on menopause care. Frustrated by the lack of reliable resources, Dr. Baumgaertel created "The Menopause Menu," a comprehensive guide that blends evidence-based medical information with holistic and integrative approaches. Read on to discover what inspired Dr. Baumgaertel to write the book, how her personal journey through menopause and rheumatoid arthritis shaped her approach, and why it's crucial for physicians to listen and advocate for their patients throughout this complex journey.</image:caption>

<image:caption>EP. 07 In today's world, the body positivity movement, especially for women and those in larger bodies, often clashes with media pressures promoting narrow and unattainable physical ideals. In my practice, I've seen patients who fit this media ideal but struggle with metabolic health, and vice versa. This raises an important question: Where does metabolic health fit into this tug-of-war? To explore this, I spoke with Dr. Matthea Rentea, an obesity medicine specialist and founder of the Rentea Metabolic Clinic. Together, we examined the complexities of health and body image, emphasizing the importance of diversifying our perspectives. Metabolic health and body positivity can coexist, but it requires us to look beyond conventional norms.</image:caption>

But today, we're diving into a different narrative with my good friend and obesity medicine expert, Dr. Ali Novitsky. Can you believe she started her first diet at just six years old? Fast forward nearly two decades and over 20 diets later, she finally uncovered the keys to living authentically in her body. Over the past 10 years, Dr. Novitsky has been teaching people the key strategies to go against diet culture and achieve long term, sustainable weight loss results. And in 2018, I was fortunate enough to be part of one of her first programs, Mind Body Marriage, which was simply life-changing. Women are often told that we can “have it all”, but the truth is that constantly chasing perfection leads to burnout and feelings of self-doubt and inadequacy. As we approach midlife, the lines between stress and perimenopausal symptoms really start to blur, leaving us feeling lost and totally unsure of which factors are truly influencing our well-being. In this episode, I’m exposing the hidden challenges faced by women trying to balance competing priorities and societal expectations. <image:caption>EP. 03 Ever since the first Women’s Health Initiative (WHI) findings were announced in 2002, women have been led to believe that hormone replacement therapy (HRT) is unsafe. The premature halt of estrogen-progestin therapy due to safety concerns resulted in a significant decline in HRT use, leaving many women to needlessly abandon a treatment that offered symptomatic relief from severe menopausal symptoms. In today's episode, we're diving deep into the WHI hormone therapy trials. We'll explore the key questions the trials aimed to answer, delve into the study's design, and dissect the findings from both the estrogen and progesterone arm and the estrogen-only arm. Additionally, we'll examine the conclusions drawn from the study, recent revisions to those conclusions, and emphasize the urgent need for further research in this area of women's health. Join me as we unravel the complexities of the WHI study and uncover the truths behind hormone replacement therapy.</image:caption>

<image:caption>Most strikingly, the estrogen-only therapy was associated with a decreased risk of invasive breast cancer across all age categories. Additionally, a decrease in hip fracture risk was observed across the board in the estrogen-only arm, accompanied by a reduced risk of all-cause mortality, except for a slightly increased risk in the 70-79 age group, the cause of which remains unclear.</image:caption>

<image:caption>Additionally, self-reported data indicated a decrease in diabetes incidence across both treatment groups. As an endocrinologist, this is significant to me because diabetes itself increases your risk of having a stroke and heart disease if your diabetes is poorly controlled. While diabetes risk decreased, there was an increased incidence of gallbladder disease, which was not surprising considering the known associations. However, the higher incidence of urinary incontinence was confusing and warrants further investigation. Reductions in hot flashes, sleep disturbances, and joint pain were also observed. Interestingly, while quality of life improved in the estrogen plus progesterone group, it worsened in the estrogen-only group. These self-reported observations offer valuable insights into patient experiences and outcomes beyond clinical metrics, albeit with some limitations in accuracy compared to objective measures.</image:caption>

<image:caption>EP. 02 Studies looking at the natural history of aging in women show that once our estrogen levels start declining and we are no longer able to reproduce to support the species consistently, the conditions that can take us out of the gene pool start to rise. In short, Mother Nature is not a feminist. But with educating and advocating for ourselves, we can still thrive. This episode gives an overview of what changes are starting and what you should look for in terms of symptoms once you hit the perimenopause transition, which can start as early as ages 35-40.</image:caption>
